Fairly typical to see this condition caused by a temperature sensor
going bad. It is on the head, adjacent to the inlet for the top radiator
hose.
When bad the ECU thinks that the engine is already up to operating temp
and turns off the cold start injector.

It could be faulty cold start injector, although failure of injector
itself is very rare occurence. It's electrically operated so it could
be bad wire/connector too.

itself is very rare occurence. It's electrically operated so it could
be bad wire/connector too.
