Why do lug nuts loosen for no known reason?

Why do lug nuts loosen for no known reason?
Twice I have had my left front wheel lug nuts loosen. The first time was just a day after my summer wheels were put back on. This was after no problems like this for 10 years of driving and changing from summer to winter wheels every year. Now it has happened again! The service folks said they have no idea. Clearly not knowing why is of no comfort to me at all. Any ideas?

Clean the mating surface between the hub and rotor, as well clean the mating surface of the rotor to wheel. Sounds like some debris has accumulated.
